The objective of the research was to evaluate the process of formulation and implementation of the Public Policy of Healthy Municipalities in Antioquia-Colombia in the period 2001-2005. The mixed method was used with a case study and a cross-sectional descriptive study. For the case study, semi-structured interviews were conducted with departmental and sub-regional officials involved in the policy formulation and implementation process, and focus groups with local, institutional, and community stakeholders receiving the policy, and for its cross-sectional study, a survey was applied. to local health directors. The results show that, despite the decision made by the departmental and local authorities to assume the policy, there were unfavorable factors such as incipient institutional development and political processes; the governmental intention without economic and technical viability and the non-inclusion of the policy in the municipal development plans. It is concluded that the process of formulating and implementing a public policy must be deliberate, argumentative, negotiated, with ethical sense, with all the actors.
